Parent Agreement
By submitting this application, (if requested) the applicant agrees to provide all standardized testing results and information for their child to ASCT and/or a third party research entity for the current year and up to six (6) years following the application date. Your agreement to provide testing information for your child to ASCT is not contingent upon your child receiving a tax credit scholarship but is based upon your application for a scholarship for your child and ASCT consideration of that request. Testing information provided will remain confidential and be used for research purposes only in determining the effectiveness of the Arizona Tuition Tax Credit Scholarship Program. Any reports generated using your child's standardized test results will be in the aggregate and will not reveal any information specific to your child. I understand that a school tuition organization cannot award, restrict or reserve scholarships solely on the basis of donor recommendation. A taxpayer may not claim a tax credit if the taxpayer agrees to swap donations with another taxpayer to benefit either taxpayer's own dependent.
